Menopause is not just hot flashes, it’s not just the end of your period. It is a process that leads to changes throughout your entire body. Perimenopause begins 7-10 years before your period stops so you can start to see changes in your entire body, your sleep and your mental health as early as your mid-30’s. We start having a decline in estrogen and progesterone. Estrogen has been shown to have a modulating effect on mood-regulating neurotransmitters such as serotonin and dopamine. And that’s going to make us more vulnerable to stress. So it’s common to see, especially in perimenopause, symptoms like: Depression Anxiety- a lot of women experience more worry, restlessness, and physical symptoms like heart palpitations. Which can also lead to more symptoms of Panic Disorder and Irritability: Hormonal changes can make women more prone to mood swings and frustration, affecting their interactions with others. It’s also very common for women to experience difficulty sleeping, insomnia, night sweats or early morning wakening- which can contribute to more irritability and that takes us to …Brain fog. Some women experience memory lapses and difficulty concentrating during menopause, which can be really frustrating. And this is all in addition to the common things people think about when it comes to menopause, hot flashes, night sweats, and metabolism changes which can make your body change quickly, in energy levels or weight gain. Did you know that 30 years of practicing Gynecology gave me the ability to immediately recognize a woman with testosterone deficiency? In fact I can usually tell if a woman is deficient in testosterone by giving her a comprehensive 10 second visual evaluation—with her clothes on! Even though it is a skill I initially learned in medical school, I further perfected my diagnostic observations by evaluating thousands of women who had symptoms of testosterone deficiency who also gave me their lab values to confirm my diagnosis of testosterone deficiency. This process further confirmed the physical signs of testosterone deficiency that accompanies aging and these physical signs are what we will discuss today. My purpose is not to teach you to be a doctor, but to help you understand yourself and your friends who are suffering because of lack of testosterone. You may have heard of the author Malcom Gladwell who proved, that becoming an expert requires 10,000 hours of practice, so this is not a shortcut to becoming a hormone expert. My goal is to wake up your mind to the visual signs of testosterone deficiency, every time you see someone you know who complains of her low testosterone symptoms.

He begins by explaining the different types of estrogens—estradiol, estrone, and estriol—and their varying roles in the body. Estradiol, the most potent estrogen, plays a critical role in regulating metabolic functions such as fat distribution, glucose regulation, and inflammation. Menopause leads to a dramatic decline in estradiol levels, which significantly impacts a woman’s metabolic health, including insulin sensitivity, fat storage, and inflammation, while increasing the risk of type 2 diabetes and other metabolic disorders. The loss of estradiol causes fat to redistribute from healthier storage areas, like the hips and thighs, to the abdominal region, where larger fat cells become more insulin-resistant and pro-inflammatory. This shift contributes to systemic inflammation and elevates the risk of cardiometabolic diseases. Ben explains how HRT, particularly with estradiol, can mitigate these effects by improving insulin sensitivity, reducing inflammation, and promoting healthier fat storage patterns. He also addresses historical concerns about HRT, including risks of blood clots and cancer. Dr. Bikman explains that oral estrogens may slightly increase clotting risks due to their first-pass metabolism in the liver, but transdermal delivery methods like patches and gels do not carry this same risk. Regarding cancer, he highlights how older studies, such as the Women’s Health Initiative, overstated the connection between HRT and breast cancer, particularly when using estrogen-only therapies. Ben emphasizes the importance of evaluating the absolute risks, which are relatively small.
