specialty in female pelvic medicine reconstructive surgery Um my niche in 1:14 urology is really about sex and genitals and pelvic floor Okay i deal with sexual 1:21 issues I deal with genital issues issues Um and my practice is based on my own my own 1:52 practice which I'm very proud of I'm probably most proud of that very few of us doctors are owning our own practice these days and it is based in Chicago 2:00 Illinois It's called Down There Urology Um and in my practice I see all adult 2:05 individuals no matter what's between their legs I do tend to see more vulva and vaginal owners um than anything else 2:12 And I talk a lot about vaginal health and sex and pelvic floor stuff And 2:18 that's what we're here to talk about I love that Now I'm not going to put anything past our listeners Can you just 2:23 kind of describe the difference between the vagina the vulva and the pelvic 2:28 floor because some of us we've heard of it We don't know what it is And so I just want us to learn together in 2:34 community with no shame I agree totally I mean no one teaches us these things I don't think I I've ever heard the term 2:40 vulva until well well into medical school maybe even early residency But so 2:47 that everybody's on the same page Technically what we see on the outside externally the female part is the vulva 2:53 Okay we always refer to it as we kind of refer to everything as vagina but the vulva is all the parts we see The it 3:00 includes the the lips um the opening of the vagina the opening of the urethra 3:05 where we pee out of It includes the clitoris All of that is the vulva Okay the vagina is the inner tube It's the 3:12 part where we things go in and out Okay it That is the part we can't actually 3:18 see our own vaginas All right so if you're pointing down there and you're like "My vagina," you're really the 3:23 inner part is the vagina Okay we can see the medical doctors can see your vagina with the specula All right so that's the 3:30 difference The pelvic floor is a group and collection of paired muscles that 3:35 envelopes are genitals and wraps around towards and envelopes our anus Um and 3:41 pelvic floor exists in penis owners and vulva owners Okay And they're important 3:46 The pelvic floor is important for the three Ps as I like to say it Peen pooping and pleasure I know That's right 3:53 girl Not the three Ps Well my pelvic floor is uh more curious now that we've 3:58 gotten all of that out of the way So talk to me about I mean you're dealing with topics many women shy away from 4:05 obviously right why do you think conversations about vulva vaginal health 4:10 and pleasure are still so taboo even in 2025 you know we vagina owners have to live 4:20 in let's say a massage Can I stop you right there you continue to say vagina owners Talk to me about that I'm 4:28 intentional when I say that I know I want to know about it Yes Because not all vagina owners are women Uhhuh Okay 4:35 You see what I'm saying so if I say women then I'm excluding vaginal owners that identify as male or or male Okay so 4:43 I'm very intentional um the individ and and so I will be referring to um v 4:48 vaginal owners Now occasionally I'll say female and male That's more for like medical biological differentiation 4:54 differentiation but I want to be very inclusive I'm very inclusive in um my 4:59 terminology So we love that about you Thank you for having me Okay Talk to us about taboos girl Yeah So you know we 5:06 live in this misogynistic world and so um first of all our pleasure has never been discussed or even um investigated 5:13 um and it is not been taught to us Um we because our genitals are quote unquote 5:20 hidden right for us to see our vulva we have to take a mirror and look Whereas 5:26 for penis owners to see their genitals all they got to do is look down They have to like push their stomach in a bit This is right there So it lends its fact 5:33 into this is some secret hidden compartment that you know has babies um 5:40 and you know is therefore a penis's pleasure and that's it Okay Um a lot of 5:47 us have no have don't know our anatomy Okay A lot of us have never seen our 5:53 very own genitals Okay You know sometimes I'm in the office I'm like in the mirror I'm like hi well introduce 6:00 yourself I'm like in yeah like here you go So are you telling all the listeners they got to go today and get in there 6:07 and you know introduce introduce themselves Yes Correct Hi Or if it's 6:13 been a while be like reintroduce like oh it's been a while baby girl Where you been okay No I can hear half of the 6:19 ladies already saying I don't know if I want to do that That feels that feels nasty That feels like that's a lot What 6:26 would you say to that because you know even Charlotte and Sex in the City had to have one of those moments girl But for those ladies that are like I don't 6:32 know if I want to do that Why do I need to do that well first of all you need to stop and ask yourself why does it feel 6:38 nasty to you like think about it What is we what what have we been we've been conditioned to believe about ourselves 6:44 that makes looking at your own body feel nasty right that's crazy to think your 6:51 own body inherently is nasty to look at right it's how we've been conditioned up here right that the female part is kind 6:58 of gross and it's not Okay Um so we have to de we have to like deconstruct our mind and our mentality about it And the 7:05 other thing is just important to know because there might be things down there that are um that may be dangerous that 7:12 you may that unbeknownst to you might need uh um to be taken care of to be 7:17 addressed Um I have had the unfortunate pleasure of diagnosing vulva cancer um 7:24 in individuals that had really not looked okay um and not seen it Um and so 7:30 it's a good idea just like you know dermatologists they'll do like a skin exam all over your your body You know 7:37 that includes your genitals okay you know it's easy to do Just grab a mirror 7:42 you know put it on if you get a big mirror and sit like lay it on the floor and then straddle over it spread those 7:48 lips and see you know see if there's um a new pigmentation or new spots or Yeah 7:55 that's what I was going to ask What are we looking for yeah Okay You said patient spots Yeah So I mean lumps and 8:02 bumps right um looking for things that are protruding You know there are some 8:07 pathologies where things might protrude out of the opening Um you know there are skin changes on the lips and just inside 8:15 the lips that might um be signs of dermatic conditions that affect the vulva and the vagina There's a condition 8:21 called lykan sclerosis that is um can be quite devastating left untreated but can 8:28 be very much treated um and it can start with just losing pigmentation of the v 8:36 of the labia um and then manifest into like severe dryness pain itching 8:41 cracking um you know of the vulva and the vagina And so those of us that look 8:47 may notice some changes that are like you know this is looking kind of different Or and take a look at your clitoris What does your clitoris look 8:54 like take a look under the hood of the clitoris Every hitter every clitoris has a hood that is covering it almost like 9:01 the foreskin of a man's penis those that are uncircumcised But this hood you can 9:07 retract and ideally see the glands of your clitoris Awesome Okay Good Your 9:14 face But you have you have the the muppet You have the puppet I do have a muppet So I wish And for those that are 9:21 audio we'll put this on YouTube You'll see the visual so that you get a sense She is real Girl she putting the hood up girl We're at the We're at the We're at 9:27 the auto garage We're at the We're at the vulva garage girl 9:32 Um talk to me about some common mis uh misconceptions right that women have 9:37 about their own anatomy that surprise you I mean I'm no longer that surprised but 9:46 um believe it or not adult women may not know that they urinate out of a separate 9:53 orifice okay that there is something called the urethra that lives in our 9:58 vulva which is where you pee out of Okay i'm going to tell you a story and it's not 10:04 meant to shame but I saw an individual in my practice and she came in saying to she's peeing out of a new hole that she 10:10 never knew existed She all her life was convinced that she peed out of her clitoris That just isn't a thing Okay 10:18 And uh it turned into just a normal anatomy lesson I was like "This is your anatomy You you you know again has never 10:24 um been taught and never looked." And you know I pointed out like I you know pointed out the normal anatomy So your 10:29 urethra where you pee out of sits right above your vagina It sits literally it sweet mates with the opening of your 10:35 vagina You put your finger in your vagina and you go right above there's the urethra Okay Um and so uh that is 10:42 one thing Another thing that is even more pervasively unknown is the fact 10:48 that the clitoris that we see right here this this nubbin is not even the entire clitoris In fact it's only a small 10:54 portion of the entire clitoris Majority of our clitoris lives underneath the tissues Our clitoris is shaped like a 11:01 wishbone okay and has like two two sets of legs that extend down from the head 11:07 okay so you have this head and then you have two sets of legs that extend down I wore a clitoris necklace for months 11:14 around and I would ask random strangers all walks of lives um uh men and women 11:22 hey what do you think this is and literally 95% of people got it wrong 95% 11:29 of people couldn't tell me what it was Okay So vast majority of adults have never seen what the actual re full 11:35 clitoris looks like Um and certainly women um don't know this either Okay No 11:40 girl I'm Girl I'm learning too I love Wishful I love Thanksgiving honey Let's get it together Awesome Awesome Awesome 11:47 So following up on that let me say one thing Yeah One the other thing is your 11:53 clitoris The clitoris is think of the clitoris like a penis If you think of the clitoris like a penis then it all 11:58 makes more sense Okay the clitoris is our version of a penis All right and the 12:04 penis the head of the penis is very sensitive right okay the head of the clitoris is very sensitive If you want 12:11 to get a penis to orgasm you might focus on the head right mhm If you want to get 12:18 a vulva owner to orgasm you focus we don't focus on the head We 12:24 are focusing on other parts Like why aren't we focusing on the head this is where we how how 90 almost 90% of us 12:30 orgasm is here Let's talk about the orgasm gap because there is one right 12:36 and I you're a huge proponent of making sure that women are having their pleasure each and every time that they 12:42 want pleasure So can you talk a little bit about that and you know I know that there are some girls that are still 12:47 faking it I know you know and you know I'm not a fan of that But how can we 12:53 start to kind of narrow that gap but also advocate for our own pleasure uh 13:00 early on in dating but also in sexual experiences in general 13:06 Number one you have to own your orgasm I believe you cannot expect somebody to 13:13 figure out what it is that gets you off Okay you may learn from sexual experiences with others like "Oh my god 13:20 that guy that guy did that and baby I liked it." Okay but you have to do I am 13:26 a big proponent of self-exloration Okay when I see individuals who rarely or 13:31 never get orgasms it's a lot of self-work It is a lot of self-work Okay you need to I give them masturbation 13:38 homework I give them sex toy homework Can you get can you give the listeners some masturbation and sex toy homework 13:43 as well please doctor so I truly believe if you don't like sex toys it's probably 13:48 just because you have not met a sex toy that you you have not met the right sex toy for you Okay and everyone is 13:54 different right what might get me off and I love everybody's crazy about this rose Not a huge fan of the rose 13:59 personally but I love it for those people who love it But other things may be So you I would explore different toys 14:07 different feelings Maybe you like pressure Maybe you like vibration Maybe you like wetness and licking Maybe you 14:13 like squeezing Maybe you you don't maybe you prefer um the focus be on your 14:19 nipples and not your genitals and that gets you orgasming Okay Um you you are 14:26 not going to become addicted or conditioned or numb or whatever to toys 14:32 toys aren't going to ruin you Okay I think that's such an important point to make because I think sometimes there are 14:38 some women women that feel like if I do it too much I'm never going to find pleasure with another person and that's 14:44 not necessarily the case So I'm glad that you're pointing that out But you talked about you talked about the exact 14:49 opposite is shown when you do the exact opposite happens tends to happen Those who do explore with toys and to do use 14:56 those are more likely women are more likely to reach orgasm and to have orgasm than those who do not Well we 15:04 love to hear that girl We love to hear that Um you talked about the rose as a a 15:11 popular toy Do you have any other names that are quite popular that people who maybe don't know the space can look up 15:18 and see and just as a starting point to explore yeah So there are vibrators that just 15:25 focus on the clitoris There are vibr vibrators that focus on like the vagina G-spot type area There are vibrators 15:32 that do both There are vibrators that focus on clitoris vagina and anus Okay 15:38 There are um and so there are yes the fully baby Okay Um so traditionally when 15:46 people think about vibrators they just think about this like phallic looking thing that you know that they can stick 15:51 in their vagina which may work great for some and may not be adequate for others Like I said almost 90% of us need 15:56 clitoreral stimulation to get one orgasm Um there are C vibrators I'm a fan of a 16:02 C vibrator Um it looks like a C or like a U that's on its side And the and 16:08 they're dual vibrators Okay So they will vibrate or suck or both typically on the 16:14 top and then vibrate on the bottom And you can put the bottom in your vagina 16:19 right just in your vagina And the top one sits right on your clitoris And you can choose to have suction or vibration 16:26 on the clitoris while you simultaneously get vibration on that vagina that upper wall of that vagina the upper wall 16:33 because remember I said the clitoris has legs that are under that upper wall of the vagina tends to be a very sensitive 16:39 spot too because it's it's in contact with the inner part of the clitoris So baby that dual vibrator 16:46 I didn't I didn't know that they sucked now girl I might have to look something up now Hello 16:54 But suction is tea It is tea baby I know that's right So it's like a vibrator It's a C-shape or a U-shape Are there 17:01 specific brands that you like or you There's a lot of them There's tons of brands I um I'll tell you the brand that 17:08 I like And full disclosure they paid me but not I like them Even if they didn't pay me like I use 17:14 them Like I love multiple of their stuff and I really like them Blesa Boutique Um 17:19 they have C the C-shaped vibrators that suck or lick or vibrate Um and then the 17:25 bottom uh the clitoris and then the bottom vibrates and you can change the intensity of both um le like you know so 17:31 they both they have separate settings So if you like well I like the feeling of 17:37 the if you like the feeling of the vibrator in your vagina but you don't necessarily want it to vibrate you don't 17:42 have to but but you want the part to suck or vibrate you can do that So like you can change it up you can switch it 17:48 up It's very like customizable if you will Yeah Um and those um the one that I 17:54 love is called the Airvibe Um and that is my favorite Air sales just went up 18:00 200% Let's go Yes Okay Okay I want to get in I want to 18:06 talk about pH balances because I know that a lot of us might not be as aware 18:12 that some of the products that we're using might not be so great in terms of pH balance or not whether it's loops Can 18:19 we talk a little bit about that because I I think that page balance actually isn't talked about as much as I think we 18:25 need to talk about it So I'd love for you to share with the ladies Girl what should we be putting down there what 18:30 shouldn't we be putting down there yeah So the pH val pH of our vagina is really 18:36 important It is um it it tends to want to be what we call more acidic which is 18:43 a good thing Okay The pH we want it to be on the lower end Um when the pH gets 18:49 off then we can be prone to um uh vaginal odor number one Okay Um we can 18:57 be prone more prone to vaginal infections like BV or bacterial vaginosis We can be more prone to 19:04 urinary tract infections or bladder infections Okay Um and so it can really just wreak havoc on our what we call 19:11 genital urinary sense um um um area um things that could throw off our pH 19:17 balance Well our pH balance can gets thrown off as we age especially with estrogen uh levels declining And so 19:25 there is a known dec um there's a no known change in pH as women go through 19:30 pmenopause and beyond And that increases their risk of all of these infections 19:35 and um you know there are things that we can do to help with that PH can be thrown off by by semen Okay So semen um 19:44 can throw off your pH That's why some individuals may notice gosh whenever I have sex especially when he ejaculates 19:49 in me then I you know have itching and odor and other issues Okay So it may be 19:54 that your the pH of your your man's is throwing you off All right Um pH can be 20:00 thrown off by products that we use like you were saying All right things that you know certain cleansers or products 20:06 that we do can disrupt our pH and actually make us more prone to 20:13 opportunistic infections All right you know these products are meant to cleanse or make us feel smell like flowers but 20:20 they can you know in some cases throw off our pH So it's really really important that we are choosy with the 20:26 products that we use down there Okay Um we want to look for things that are pH 20:33 balanced We want to look for things that aren't having too many like strong o um 20:39 smells Um um and that you know just are 20:45 less um likely to um make our um pH get off the acidic and and go more towards 20:51 basic Have you seen any products just like in your practice that you're like "Oh my god this person's using this and 20:57 it's like it throws them off." Are there any patterns that you've seen or is it just really different just based on the body it's that's hard to like even 21:04 create those patterns So I will say this I mean I think over the last 10 years or 21:09 more there is a complete market of products for the vulva and the vagina 21:16 keeping in mind pH balance Okay Back in the day like I'm old enough like back in 21:21 the day these summer's eaves and whatnotss they were not they weren't doing good They weren't good for us Okay 21:28 Um but people are you know individuals are not really using those products anymore thankfully Um and the products 21:34 we now have on the market are m much more thoughtful um and careful um and 21:40 and and you know uh every individual might have individual like preferences but for the most part I'm seeing uh 21:47 these newer vaginal cleanser products being um um better suited Having said 21:53 that to be fair you don't need a fancy fancy cleanser You really don't Your 21:58 vagina itself is a self-cleaning organ So to the thought of cleaning out your vagina is actually quite ridiculous Okay 22:05 we don't recommend it People might want to clean their vulva Okay you could 22:11 simply let water run warm water run over your vulva and you know um cleanse it that way But if you want a gentle 22:18 cleanser I think there are the most of the current products out there that 22:23 you'll see on the shelf or online keep in mind pH balance So I have not run 22:28 into a bad actor in a very long time Okay that's good to know Now you had 22:34 mentioned a little bit earlier about menopause Yeah premenopausal And I want 22:39 to talk a little bit about that because I have a lot of women that are in that era So I would love for you to share 22:44 just kind of what that means for one's vulva vagina and if there are any 22:50 differences when it comes to women of color versus other women 22:56 Wait this could be its own separate Well girl let's get into at least a little bit honey cuz the girls got to know They 23:02 got to know They got to know you Please listeners I need you to understand that 23:08 as we get older per menopause is typically in our 40s Menopause typically 23:14 happens in 50 51 and then everything after that is postmenopausal And so as 23:20 we get older starting as early as in our 40s sometimes late 30s our vagina and 23:25 vulva lose estrogen And what that does is it actually changes how it looks and 23:32 how it feels and how it functions Okay it changes how it looks The labia inner 23:37 labia may shrink The clitoreral hood may get stuck the um you may lose 23:43 pigmentation Um it may actually shrivel the opening of the vagina Okay How it uh feels It 23:53 feels drier It may feel itchy It may feel hot or burning It may feel raw like 24:00 sandpaper How it functions it loses its ability to lubricate on its own So you 24:06 may be arousing your brain but nothing's getting wet down there Okay it loses the ability to um to expand and to widen So 24:16 an aroused um well estrogenized vagina actually is looser because we have these 24:21 these things called rug that and we have muscle that expands and so an aroused vagina widens and actually gets longer 24:29 like tremendously longer Okay that allows us and prepares us for penetration right mhm And um with 24:36 menopause pmenopause that ability to do that and get ready for penetration go um 24:42 gets weaker and may go away And so our vaginas essentially become this like stiff dry irritated pipes and the vulva 24:51 is this irritated opening None of that is fun for sex So 50% of people of 24:57 menopausal women have pain with sex 50 Wow 25:02 Okay we're talking about women as early as 40s okay you know these are not old 25:08 old people okay all right Um and almost 90% of us will have bothersome 25:15 noticeable vaginal dryness Okay one out of three of us will get 25:20 recurrent UTI because it increases the risk of that the pmenopause and menopause Okay We have all of this havoc 25:27 that is being wre in our lady parts And the most important 25:33 thing to remember is that we don't have to suffer like this Okay in my world 25:39 perfect world vaginal estrogen which is the holy grail to help combat everything 25:45 I've just said would be preventative care You are 40 Okay Fenna Milhouse you 25:52 have entered pmenopause just so you know you are you know very likely 90% um of 25:59 people have bothersome symptoms You know um but you don't have to you don't have to get that way We can start you on 26:05 vaginal estrogen It's very safe It doesn't get into your bloodstream It's not going to increase your risk of breast cancer It's safe to use if you're 26:11 a smoker It's not going to increase your risk of blood clots It's not going to um throw off your periods It's not going to 26:17 affect your man It's all of these things And it can help preserve your vagina and 26:22 your vulva Okay it can help preserve your walk It can help preserve your um 26:28 uh decrease your risk of infections Like sign me up today Okay if men had this 26:36 happen to them it would be like my friend says a vaccine Okay if penises 26:42 had the same thing happened to them there would be a vaccine All right every 26:47 man would know Oh yeah Yeah Yeah bro you gotta get this You got to get your you know your penis estrogen because da d d 26:53 d d d d d d d d d d d d d d d d d d d d d d d d d d d d d d d d d d d d d d d Okay it is um very 26:59 very setting for me as a physician to see the disparity that takes place when 27:05 we talk about men's health and women's health And then you you mentioned something about black women that's even 27:11 a double disparity right okay there's already a gender disparity now a race 27:17 and gender disparity Black women biologically are more likely to enter 27:23 into menopause earlier and to deal with menopause Yes And to 27:28 deal with menopause longer Okay Much longer So we are we are so 27:36 like you know three four five years longer Okay Black and Latina women Okay 27:43 Um so we are you know more likely to suffer if you will have more you know 27:49 but less likely to be offered or to use 27:54 any form of hormone replacement therapy um um or get treated 28:01 and probably as we know with medicine less likely to be taken seriously You got a black woman who's complaining 28:07 about vaginal problems and it's you know unless it's an STD or AIDS or some other thing you know they get swept they can 28:15 get easily easily swept and ignored and under the rug So it is um because I just have seen so much suffering so much So for the listeners that are like damn I need to get on this 28:38 estrogen We just need to go to the doctor and tell them I need some estrogen and I need it now Like what's that process look like and how can we 28:44 advocate for our our health yeah great question Okay So first of all vaginal 28:50 estrogen is not the same as like hey I want HRT for like my hot flashes I say 28:56 that to mean honestly any vulva or vagina can get vaginal estrogen safely Okay Um you don't have to have gone you 29:03 don't have to be like well I'm not menopausal yet Do I can get can I get it technically you can Now will you find a 29:09 physician who is up to date and educated on this hot or cold but the what you say 29:15 is you you come in armed with education You say listen I am having um I am you 29:21 know um want to I want to um improve my 29:27 vaginal health You may be having symptoms or I'm having vaginal dryness or it kind of hurts when I have sex or 29:32 I'm itching and I know that vaginal estrogen is safe to use I know that there are very few contraindications 29:38 really none Um and that it um does not need to require there's no blood test 29:44 that you need before Okay There's no blood test you need to monitor Okay you could still be menrating and get vaginal 29:51 estrogen Um it's even safe in breastfeeding moms which by the way breastfeeding moms have low estrogen and 29:57 so they look like they can look like menopausal women Okay Um but anyway and 30:03 you you just say "Hey I know it's safe."
